CHWs service is informed through the lived experience of the CHW and their families, social circles, and communities.\n \n \n ESSENTIAL DUTIES AND RESPONSIBILITIES \n \n Assisting clients with completing applications for housing, rental assistance, benefits, and supportive services. \n \n \n Collecting, organizing, and maintaining required documentation such as identification, income verification, benefits letters, and rental paperwork in line with program and grant standards. \n Assisting with program workshops related to tenancy education, independent living, and financial literacy in partnership with community providers. \n Preparing reports and assisting with data collection to demonstrate client progress and program outcomes. \n Serving as a point of coordination between clients, Case Managers, and the Housing Navigator to ensure timely communication and resolution of housing-related matters. \n Providing general administrative support including tracking deadlines, maintaining records, and supporting program communications. \n Upholding trauma-informed, client-centered, and culturally sensitive practices in all interactions while maintaining professional boundaries and confidentiality. \n Contributing to continuous program improvement by identifying barriers, sharing feedback, and supporting strategies that strengthen housing services. \n Maintain all data and documentation of housing program in relationship to audit readiness \n Maintaining client files, case notes, and data entry across ECW system to ensure accuracy and confidentiality. \n Supporting the Housing Navigator with rental assistance documentation and payment requests in coordination with the Finance Department. \n \n \n \n \n Responsibility to Team – actively support assigned team/department by participating in and supporting all team/departmental initiatives, proactively communicating with supervision, meeting assigned goals and objectives, and committing to embracing process improvement initiatives.\n \n Responsibility to Organization – actively support SJCH by consistently meeting St. John’s Mission, Vision, and Values; modeling positive interactions with patients, staff, and community; keeping informed of internal (e.g., policies and procedures) and external (e.g., legal or political matters) which may affect business.\n \n Additional Duties – as assigned.\n \n For Grant Funded Positions – if position is grant funded, grant specific duties which may be in addition to the above are attached with employee attestation herewith.\n \n St. John’s Community Health is an Equal Employment Opportunity Employer\n \n \n QUALIFICATIONS\n \n To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Ability to interpret situations, characteristics, behavior, etc. and respond appropriately. \n \n Computer Skills \n To perform this job successfully, an individual should have knowledge of MS Outlook, MS Excel, MS Word, and Internet software. CHWs service is informed through the lived experience of the CHW and their families, social circles, and communities. \n The role requires strong coordination, documentation, and operational oversight to meet participant needs and funder requirements.Program Oversight\n The Peer Community Health Worker is a comprehensive reentry initiative intended to reduce recidivism and improve social functioning among justice-involved individuals.
