Position Summary &amp; Responsibilities:
<ul>

Evaluate, plan, and implement individualized PT treatment plans for SNF residents
</li>
<li>
Provide therapeutic interventions to improve mobility, strength, balance, and function
</li>
<li>
Document patient progress and update care plans in compliance with facility standards
</li>
<li>
Collaborate with interdisciplinary team (RN, OT, SLP, MD) to support patient outcomes
</li>
<li>
Educate patients and caregivers on mobility, safety, and rehabilitation goals
</li>
