Location: Concord, CA - Hybrid. This is a field-based home health position serving patients in their homes throughout the surrounding territory, with documentation typically completed in the field.
Note: Candidates must have an active California Physical Therapist (PT) license. Home health experience, HCHB/Homecare Homebase, OASIS experience, BLS/CPR certification, and prior field-based patient care experience are preferred but not required.

Job Description
- Provide physical therapy services to patients in their homes.
- Evaluate patient mobility, strength, balance, and functional needs.
- Develop individualized treatment plans focused on improving independence and quality of life.
- Deliver therapeutic exercises and interventions designed to improve mobility and physical function.
- Educate patients and caregivers on exercises, safety, and strategies to support continued progress.
- Monitor patient outcomes and adjust treatment plans as appropriate.
- Collaborate with other healthcare professionals involved in the patient's care.
- Complete accurate and timely clinical documentation.
- Travel throughout the assigned territory to provide patient care.
Qualifications
- Active California Physical Therapist (PT) license required.
- Home health Physical Therapist experience preferred.
- HCHB/Homecare Homebase experience preferred.
- OASIS experience preferred.
- BLS or CPR certification preferred.
- Experience providing field-based or in-home patient care is a plus.
