Responsibilities
- Lead coordination of patient care with the care team, ensuring the interdisciplinary plan aligns with the patient’s clinical course and ongoing needs.
- Monitor diagnostic testing, treatments, procedures, and other care elements to maintain alignment with the care plan.
- Discuss with physicians the appropriateness of resource utilization, consultations, treatment plans, estimated length of stay, and discharge planning.
- Coordinate transfers to tertiary centers and ensure the transfer includes necessary information for continuity of care.
- Focus on complex patients, including high ED utilizers, chronic pain, substance use, and homeless patients, and collaborate with other case managers to ensure appropriate ongoing care.
- Collect data and support regulatory and accreditation requirements related to case management activities.
Requirements
- Current RN license in the state of California
- Basic Life Support (BLS) certification required
- Case management certification preferred
- Bachelor’s Degree in Nursing (BSN) preferred
- At least two years of recent experience in a care management or case management role
- Ability to work effectively in a hospital-based care coordination environment
