Role Description The Registered Nurse role is a full-time, on-site position serving patients in their homes throughout Los Angeles County, CA. The RN will provide direct patient care, including assessments, medication administration, wound care, and monitoring of vital signs and symptoms. Day-to-day responsibilities include developing and updating care plans, collaborating with physicians and other healthcare professionals, and educating patients and caregivers on treatment, safety, and disease management. The nurse will document all care in accordance with regulatory and agency standards, respond promptly to changes in patient status, and participate in case conferences as needed. This role also involves coordinating visits, maintaining professional boundaries, and ensuring that care is delivered in a respectful, culturally sensitive, and compliant manner.

- Active Registered Nurse (RN) license in the state of California and current Basic Life Support (BLS) certification
- Experience in home health, acute care, or related clinical settings, with strong patient assessment and care planning skills
- Ability to perform key nursing procedures such as medication administration, wound care, and management of chronic conditions
- Strong communication and interpersonal skills for interacting with patients, families, and interdisciplinary care teams
- Reliable transportation and willingness to travel within Los Angeles County to provide in-home care
- Solid time management, organization, and documentation skills, including familiarity with electronic health records
- Commitment to patient safety, infection control practices, and adherence to all regulatory and agency policies
- Preferred: Experience with pediatric or adult home care, bilingual language skills, and advanced certifications relevant to home health
