Primary Care Physician - PACE / Geriatrics / Value-Based Care
Location: Modesto, California
Job Type: Full-time
Schedule: Monday through Friday, 8:00 AM-5:00 PM, including home visits
Call: Rotating phone-only on-call schedule a few nights or weekends per month
Compensation:
Base salary: $250,000-$280,000 annually
Quarterly bonus offered
Estimated total annual compensation: $315,900-$386,600

Avento Health is partnering with a mission-driven healthcare organization seeking a Primary Care Physician to provide comprehensive, community-based care for older adults in a PACE setting.
This is a rare opportunity to practice value-based, holistic primary care with the support of an interdisciplinary team, an average of about 8 patients per day, no hospital rounds, and the ability to build meaningful long-term relationships with a vulnerable senior population.

Key Responsibilities
- Perform comprehensive in-person assessments for participants in a community-based senior care model
- Lead and collaborate with the interdisciplinary team to develop individualized care plans
- Provide ongoing primary care in a PACE and value-based care environment
- Coordinate discharge planning for acute, short-term, and long-term placement needs
- Deliver high-quality, holistic care across home, clinic, and community settings
- Maintain accurate and timely clinical documentation
Required Qualifications
- MD or DO from an accredited medical school
- Successful completion of an ACGME-accredited residency program
- Active California physician license in good standing and without restrictions
- DEA registration and state CDS certification
- Eligible for enrollment to provide services to Medicare and Medi-Cal participants
- Must obtain and maintain CPR and First Aid certification within 180 days of hire
Preferred Qualifications
- Board certification preferred
- Training or experience in Internal Medicine, Family Medicine, Emergency Medicine, or Geriatrics
- Experience caring for senior, medically complex, or Medicare populations preferred
- Commitment to patient-centered, community-based care
- Strong clinical judgment, communication, and care coordination skills

Work Environment
This role includes a combination of office-based and field-based work in the Modesto area, including participant visits in home and community settings. Physicians in this model work closely with an interdisciplinary care team and have the opportunity to make a direct, lasting impact on older adults receiving coordinated care through a PACE program.
Additional Employment Requirements
Proof of vaccination or a signed vaccine declination form may be required prior to employment, consistent with the employer’s infection control and workplace health policies. Team members are expected to follow applicable PPE and vaccination-related standards.
