Overview
We are seeking a dedicated and compassionate Physical Therapist to join our team in Pleasant Hill, California, in a permanent full-time position. Responsibilities

Evaluate patients and develop individualized physical therapy treatment plans

Provide skilled physical therapy interventions based on patient needs and goals

Help patients improve strength, mobility, balance, coordination, and functional independence

Monitor patient progress and adjust treatment plans as appropriate

Document evaluations, treatments, progress, and outcomes accurately and timely

Collaborate with nursing, occupational therapy, speech therapy, and other members of the interdisciplinary team

Educate patients and caregivers on therapeutic exercises, mobility strategies, and safety techniques

Follow facility policies, professional standards, and applicable regulations

Participate in care planning and contribute to positive patient outcomes
Qualifications

Doctor of Physical Therapy (DPT) or equivalent accredited physical therapy degree

Current Physical Therapist license in California or eligibility for licensure

Strong clinical, communication, and interpersonal skills

Commitment to providing compassionate, patient-centered care

Ability to work effectively both independently and as part of a multidisciplinary team
