PATIENT POPULATION:

The patient population served is all patients, including geriatric, adult, adolescent, pediatric, and newborn. This also includes services which affect facility staff, physicians, visitors, vendors and the general public.

POSITION SUMMARY:

Under the supervision of the VP of Quality, Regulatory and Compliance Affairs, the Risk & Patient Safety RN is responsible for duties related to patient safety analysis and systems review. This includes, but is not limited to, performing root cause analysis, management of the event reporting system, and systems review. Supports high quality, safe care by evaluating systems and processes within the organization. This person is responsible for communicating, maintaining and processing defined or designated confidential Patient Safety information, including assisting with Hospital-wide and Medical Staff Peer Review activities. Duties also include data collection via chart review, analysis, recommendation, implementation, trouble-shooting and support for any given department and facility need. Will assist the VP with educational opportunities for staff related to patient-safety principles. In addition, this position assists with processing and analyzing potential claims and assists staff in identifying and mitigating risk-related issues, coordinates the Grievance Committee, and prepares reports as assigned. Must be professional and cooperative with patients, visitors, employees and members of the medical staff, in support of the hospital's customer service goals.

Must be able to work normal/scheduled working hours to include Holidays, call-backs, weeknights, weekends, and on-call. Agrees to participate, as directed, in emergencies and community disasters during scheduled and unscheduled hours. As a designated disaster service worker you are required to assist in times of need pursuant to the California Emergency Services Act.

Your position has been defined as exempt (Exempt employees are paid on a salary basis as their duties may include more complex tasks that require them to work inconsistent or longer hours on a weekly basis. Exempt salaried employees also may be obligated to work as many hours as required to fulfill their responsibilities) therefore you may have the ability to work remotely as long as your VP has given prior approval. EDUCATION/TRAINING/EXPERIENCE:

Must have BSN degree or obtain within 18 months of hire date. At least two years’ clinical experience in acute care facility as a Registered Nurse is required. A minimum of 3 years’ experience in Risk/Patient Safety principles and related activities is required. Demonstrates independent judgment, autonomy and initiative in leadership skills, time management and organizational skills and the ability to prioritize projects/functions in a busy work environment.

Proficient in reading and interpreting various documents, including safety guidelines, operational procedures, maintenance instructions, and manuals. Capable of preparing routine reports and correspondence as needed. Ability to speak and write in English, with the ability to write clearly, comprehend verbal and written instructions, and follow directions accurately.

Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, and percentages if required.

Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.

To perform this job successfully, an individual should have basic computer skills.

LICENSURE/CERTIFICATIONS:

Must have California current RN License. Must have and maintain a current BLS certification or obtain within orientation process, prior to hands-on patient care.

ESSENTIAL FUNCTIONS:

  • Regular, punctual attendance and physical presence at the assigned work location and workstation, ready to perform job duties at the scheduled start time.
  • Assists the VP of Quality Regulatory and Compliance with coordinating and conducting root cause analyses related to quality and patient care throughout the organization, including identifying contributing factors and recommending opportunities for improvement.
  • Oversees of the district’s event reporting system, including assigning events to the leader for investigation/review, monitoring the progress of investigations, identifying potential patient safety trends, and Following up on the implementation of recommended corrective actions.
  • Assists the VP in Developing and evaluating corrective action plans to address identified patient safety risks and opportunities for organizational improvement.
  • Interprets and applies applicable regulatory standards and demonstrates current knowledge of local, state, and national standards/codes/regulations To support organizational compliance and patient safety initiatives.
  • Demonstrates competence in and applies Just Culture principle and systems theory When reviewing patient safety events, evaluating organizational processes, and identifying opportunities for improvement.
  • Receives and reports to medical staff, issues related to physician-related concerns brought forward through the district’s dedicated platforms, to be considered for Peer Review through the designated Medical Staff processes.
  • Coordinates the claims process by ensuring timely and comprehensive responses to claims and litigation-related requests, including gathering and reviewing relevant documentation. Interfaces with claims administrators, legal representatives, and appropriate district personnel as necessary.
  • Serves as the coordinator of the Grievance Committee, ensuring the timely resolution of grievances as per policy and regulatory guidelines
  • Maintains legal files, ensures that charts in litigation or investigations are secured
  • Administers and maintains the district’s current DMV pull notice program; monitors enrolled drivers’ records to ensure they are within acceptable standards as per policy and Reports findings and coordinates appropriate follow-up actions when necessary.
  • Serves as the primary resource for staff regarding risk-related questions and mitigation techniques, providing guidance on identifying, reporting, evaluating, and mitigating organizational and patient safety risks.
  • Leads Patient Safety rounding throughout the organization. Evaluates identified concerns, communicates findings and recommendations to appropriate department leaders, and monitors implementation of corrective action plans.
  • Collects, analyzes, and monitors patient safety and risk-related data to identify trends, evaluate potential areas of concern, and recommend opportunities for improvement.

OTHER FUNCTIONS:

  • Assists the VP with the coordination of activities related to actual and mock surveys.
  • Participates in the development of policies and procedures, which will promote compliance with regulatory/accrediting agencies.
  • Facilitates and monitors progress with multidisciplinary Quality /Regulatory projects as requested.
  • Organizes time and workflow to complete job responsibilities in an accurate and timely manner.
