Job DescriptionPhysical Therapist (PT) Home Health (Per Diem) Serving Alameda, Contra Costa, Marin, Napa, Sacramento, San Francisco, San Joaquin, San Mateo, Santa Clara, Santa Cruz, Solano, Sonoma, and Stanislaus Counties Job Description We are looking for a licensed Physical Therapist (PT) to provide home health visits to patients in their homes. The PT will evaluate patients, develop care plans, and provide therapy to improve mobility and function. Responsibilities

<li>Perform patient evaluations</li>
<li>Provide physical therapy treatments</li>
<li>Develop and follow care plans</li>
<li>Educate patients and caregivers</li>
<li>Complete documentation on time</li>
</ul>
Requirements 
<ul>
<li>Active California PT License</li>
<li>Home health experience preferred</li>
<li>Valid drivers license and reliable transportation</li>
<li>CPR certification</li>
