Role Summary
The Caregiver II provides compassionate, hands-on personal care and support to patients receiving home care and/or center-based services. Under the direction and supervision of a Supervisor or Manager, this role supports patient dignity, safety, and independence while ensuring care is delivered according to individualized care plans.
In addition to direct care, the Caregiver II plays a leadership role by supporting onboarding and ongoing training of fellow caregivers. This position is ideal for an experienced caregiver who enjoys mentoring others and delivering high-quality, patient-centered care in a collaborative healthcare environment.
Duties

Provide personal care and escort assistance in accordance with individualized care plans, including: Grooming, dressing, peri-care, bathing, showering, and toileting, Housekeeping, laundry, meal preparation, feeding assistance, etc.

Observe and report changes in patient condition to the team in a timely and professional manner.

Demonstrate safe care practices by using proper body mechanics, adhering to universal precautions, and maintaining a safe environment.

Interact professionally with patients, families, and team members, encouraging engagement in conversation and activities.

Perform all duties of a Caregiver I as needed.

Support onboarding and ongoing training of Caregiver I and Caregiver II staff using established training materials and manuals.

Actively participate in departmental meetings and family meetings as required.

Utilize safety equipment appropriately for lifting and transfers; provide positive guidance to others on proper use.

Perform other related duties as assigned.
Required Qualifications
Experience
-
Minimum of one (1) year of experience working with elderly or frail populations.
-
Experience working with seniors with dementia.
Knowledge, Skills & Abilities
-
Understanding of an integrated, patient-centered care model gained through experience or training.

Flexible, positive attitude with the ability to work across multiple care settings.
-
Ability to work independently and collaboratively as part of a team.
-
Strong interpersonal and communication skills.
-
Basic English reading and writing skills required; bilingual ability may be required for specific assignments and daily patient interaction.

Basic computer skills, including the ability to use or learn email and electronic health record systems.

Ability to explain and model safe lifting techniques and proper use of safety equipment.
Licenses & Certifications (Required Upon Hire)

Home Health Aide (HHA) or Certified Nursing Assistant (CNA)
-
CPR (Cardiopulmonary Resuscitation)

First Aid
Desired Qualifications
-
Bilingual proficiency (Cantonese or Spanish preferred, in addition to English).

Minimum of one (1) year of experience working in a home care or clinic setting.

Prior experience mentoring or supporting the training of caregiving staff.
