Title: Physical Therapist (PT)
Employment Type: Full-Time / Permanent
Location: Greenbrae, CA 94904
Schedule: 8:30 AM – 4:30 PM
Pay: Hourly Rate: $63-$66/hour (Based on experience) + Benefits
- $10,000 Sign-On Bonus
Facility Setting:
- Skilled Nursing Facility (SNF)
- Assisted Living (AL)
- Independent Living (IL)

-> Responsibilities:
- Evaluate patients and develop individualized physical therapy treatment plans.
- Provide skilled PT interventions to improve mobility, strength, balance, safety, and functional independence.
- Treat patients across SNF, assisted living, and independent living environments.
- Manage a diverse caseload including orthopedic, neurological, post-acute, and medically complex conditions.
- Collaborate with physicians, nursing staff, occupational therapists, speech therapists, and interdisciplinary teams.
- Complete accurate and timely clinical documentation.
- Educate patients and caregivers on therapy goals, safety techniques, and home exercise programs.
- Participate in care planning and maintain compliance with Medicare and regulatory requirements.
-> Qualifications:
- Active California Physical Therapist (PT) license.
- Graduate from an accredited Physical Therapy program (DPT preferred).
- Experience in SNF, Long-Term Care, Assisted Living, Independent Living, or Post-Acute Rehabilitation preferred.
- Strong clinical judgment, communication, and documentation skills.
- CPR/BLS certification preferred.
