Job Type: Full-Time, In-Person
Location: Porterville, California area
Compensation: $50+/hour plus benefits

Job Responsibilities:

Provide high-quality Home Health care to a caseload of approximately 4 patients daily
Conduct patient assessments, monitor vital signs, and develop care plans
Assist with diagnostic evaluations and test result interpretations
Collaborate with Medical Assistants, Nurse Practitioners, and administrative teams
Offer triage services to determine the urgency of care
Deliver specialized care for patients with memory-related conditions
Maintain thorough and accurate patient records

Job Requirements:

Active RN license in California
Home Health or Hospice experience preferred
Must have a reliable vehicle for travel
Strong assessment, communication, and critical-thinking skills
Ability to work collaboratively in a multidisciplinary team
