PURPOSE OF POSITION: The Intake/Outreach Specialist supports the Aging in Place Home Assessment process and helps implement the strategic goals and objectives of the Aging in Place Older Adult Home Modification Grant Program.

Education and Certification:

o A Bachelor of Science (B.S.) in Kinesiology or Health & Wellness, or being actively in progress toward this degree.

RESPONSIBILITIES/DUTIES:

- Assist with interviews of participants to ensure regulations and laws regarding age, income, costs, and home modifications are adhered to.
- Sets up appointments with participants and assists with the in-home environmental assessment.
- Knowledge of policies and procedures required by OAHMP grant.
- Learns to utilize Home for Life Accessibility Ratings that measure one's ability to safely perform desired daily living activities in their own home to maintain independence. Ratings are captured before and after home modifications to show the increase in safety and accessibility of the home environment.
- Support the Program Manager and Program Coordinator in developing partnerships with community organizations, medical providers, and other service providers-including Kaiser Permanente, Sutter Medical Group, and Oak Valley Hospital-to strengthen referrals for clients who need fall prevention equipment to remain safely at home.
- Provide Education/Outreach on Aging in Place program to the community.
- Provides Marketing tools, i.e., Brochures, Flyers, Power Point and other marketing materials for presentations.
- Social Media, post on social media as needed.

Part - Time Position: 20 hours per week (Monday - Friday), flexibility with school schedule.
