The ideal candidate will have strong acute care hospital Case Management experience with a focus on care coordination, discharge planning, utilization review, and working with HMOs and IPAs.

Required Qualifications

<li>Minimum 2 years of acute hospital Case Management experience</li>
\n
<li>Acute Care Hospital Case Management experience must be current within the last year</li>
\n
<li>Experience performing Care Coordination and Discharge Planning</li>
\n
<li>Experience working with HMOs and IPAs</li>
\n </ul>
\n
Required Certifications &amp; Licenses
\n
<ul>\n
<li>Valid CA RN License must be in hand at submission</li>
\n </ul>
