Administrative position responsible for coordinating, monitoring and implementing program for clients at intermediate care facilities for developmentally disabled adults and children.
- You will be lead staff of the interdiscplinary team
- Must be able to supervise direct care staff to ensure individualized care plan is implemented.
- Provide training to direct care staff on ISP goals and objectives
- Provide client case management
- Coordinate services from other professional members such as Regional Centers, registered nurse, physicians, specialists, human resources and consultants
- Conduct meetings and follow up with monthly, quarterly and annual reports
- Conduct assessments and evaluation of strengths and needs of clients
- Must be able to work independently with minimum supervision
- Take clients to medical appointments as needed
- Coordinate services with outside agencies
- 22 hours per week, salaried
