Local: $58 – $59/hour
Traveler: $60 – $62/hour

Position Overview
We are seeking an experienced MSW to provide acute-care medical social work and discharge planning services to patients and families.

Required Qualifications

Master of Social Work (MSW).
2 years of acute-care medical social work experience.
Experience performing both social work and discharge planning roles.

Responsibilities

Provide social work services to patients in an acute-care setting.
Assess psychosocial needs and identify barriers to care.
Coordinate discharge planning based on patient needs and available resources.
Collaborate with physicians, nurses, case managers, and other healthcare professionals.
Assist patients and families with post-discharge planning and community resources.
Support care transitions and arrange appropriate follow-up services.
Document social work assessments, interventions, and discharge plans.
Communicate with patients, families, and the healthcare team to support continuity of care.
