Position Title: Primary Care Call Center Outreach Agent
Department: Primary Care
Reports To: As assigned by Program Director
POSITION SUMMARY
The Primary Care Call Center Outreach Agent is responsible for health education and outreach. This position requires managing a high volume of inbound and outbound calls. Provides education and navigates patients through referrals from government and non-profit organizations to include county agencies, hospitals, medical groups, health centers, mental health and substance use treatment providers and others, and follow-up to increase awareness and utilization of Tarzana Treatment Centers services for the community.
HOURS
8 hours per day / 5 days per week
CATEGORIES OF DUTIES
- Promotes Health Education throughout the community, solicits and assesses patients’ interest in program participation and assures that patients meet the eligibility criteria as specified by the program objectives.
- Conducts structured, scheduled educational outreach sessions covering approved topics to foster awareness of healthcare services.
- Manage and navigate a high volume of inbound and outbound calls in a timely manner with excellent customer service.
- Identify patients’ needs and recommend pertinent information and referral services for medical, social, and emotional support.
- Collect and input accurate patient data into Tarzana Treatment Centers databases.
- Updates curricula for health education.
- Establishes and keeps the directory of information regarding available programs, services, contact persons, and avenues of access in the community, and trends of knowledge regarding health services.
- Assists with orienting new Health Educators to program’s policies and procedures.
- Develops and submits accurate reports regarding outreach sessions, patient information, and other information as needed.
- Performs other duties as assigned by Supervisor, Director, and/ or Administration.
EDUCATION/ EXPERIENCE
- High School diploma required.
- Valid California driver's license and insurance.
- Bi-lingual English/ Spanish a plus.
