Skilled Nursing Pharmacy in Hayward, CA is looking for a dedicated and knowledgeable Staff Pharmacist to join our team on a per diem basis. Responsibilities:
- Dispense and verify medications accurately for skilled nursing facility patients
- Review and interpret physician orders, ensuring compliance with state and federal regulations
- Perform drug utilization reviews (DUR) and monitor for drug interactions and contraindications
- Collaborate with nursing staff and healthcare providers to optimize patient medication therapies
- Maintain accurate patient medication profiles and pharmacy records
- Ensure proper labeling, packaging, and dispensing of medications including controlled substances
- Support pharmacy technicians and provide guidance on daily operations
Requirements:
- Active California Pharmacist License (RPh) in good standing
- Doctor of Pharmacy (PharmD) or equivalent degree
- Long-term care (LTC) or skilled nursing facility pharmacy experience preferred
- Strong knowledge of geriatric pharmacology and medication management
- Familiarity with California Board of Pharmacy regulations and DEA controlled substance requirements
- Excellent attention to detail, communication, and organizational skills
- Ability to work independently and adapt to a flexible per diem schedule
