Job Details:

• Day shift schedule with mileage reimbursement in addition to rate
\n• Physical Therapy licensure required in all applicable states of practice
\n• Doctor of Physical Therapy (DPT) degree required if license obtained on or after January 2016 or foreign equivalent
\n• Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS, HS-BLS, or RQIBLS) certification from approved vendor required
\n• Valid Driver's License required and must be at least 21 years of age or older
\n• Provide physical therapy services in patients' homes
\n• Travel between patient locations as part of daily responsibilities
