Director of Patient Care Services
Job Summary:
We are seeking an experienced and compassionate Director of Patient Care Services to join our team at Anchor Health in Sacramento, CA. Responsibilities:

Lead the patient care services department, overseeing the development, implementation, and evaluation of patient care strategies and initiatives

Collaborate with cross-functional teams to develop and maintain patient-centered care standards and guidelines

Foster a culture of excellence, quality, and safety, aligning with Anchor Health's mission and values

Analyze and report on patient care outcomes, identifying opportunities for improvement and implementing corrective actions

Serve as a liaison between clinical and non-clinical staff, ensuring seamless communication and collaboration

Participate in strategic planning, identifying opportunities for growth and expansion in patient care services

Develop and maintain relationships with healthcare providers, referrals sources, and internal stakeholders

Ensure compliance with regulatory requirements, accreditation standards, and industry best practices
Requirements:

2+ years of experience in a leadership role in patient care services or a related field

Strong understanding of patient care delivery models, care coordination, and care transitions

Excellent communication, collaboration, and leadership skills

Ability to analyze data, identify trends, and make informed decisions

Experience with budgeting, forecasting, and financial planning

Knowledge of healthcare regulations, accreditation standards, and industry best practices

Bachelor's degree in Nursing, Healthcare Administration, or a related field (or equivalent experience)

Experience with HomeCare Homebase EMR
