Physical Therapist
Job Summary
Evaluates and treats patients with physical impairments, injuries and disabilities to restore function, relieve pain and prevent further disability. Develops individualized plans of care and delivers therapeutic interventions. Educates patients and caregivers and collaborates with the care team.
Education & Experience
• Doctor of Physical Therapy (DPT) degree, or accredited degree in physical therapy: Required
• Physical therapy clinical experience: Preferred
Licenses & Certifications
• Physical Therapist licensure in the state of practice: Required
• Basic Life Support (BLS) certification: Required
Key Responsibilities
• Perform initial evaluations and re-evaluations and establish individualized plans of care.
• Deliver therapeutic exercise, manual therapy, gait training and modalities.
• Set measurable goals and track patient progress.
• Supervise physical therapist assistants and aides as appropriate.
• Educate patients and caregivers on home programs and safety.
• Document evaluations, treatments and discharge planning accurately.
