NP / PA - Geriatric Care (SNF - New Grads Welcome - Flexible Schedule)
Job Description
A pain management practice expanding across Sacramento-area skilled nursing facilities is hiring NPs and PAs for geriatric pain and palliative care roles. Round on the same residents weekly, build real treatment relationships, and earn $60–$100/hour plus a productivity bonus with consistent full-time hours. New graduates welcome; formal onboarding provided. Malpractice with tail coverage included. New seats added as new facilities come on board.

Key Responsibilities

- Conduct comprehensive pain assessments and physical exams
- Develop evidence-based treatment plans for acute and chronic pain
- Prescribe and manage pain medications per state and federal regulations
- Coordinate with physical therapy, acupuncture, and interventional services
- Educate patients and families on pain management and lifestyle strategies
- Monitor outcomes through follow-up visits and adjust treatment plans
- Document accurately in TEBRA and PointClickCare
- Build strong relationships with SNF leadership and staff

Qualifications
\n
- \n
- Active, unrestricted California NP or PA license
- AGACNP-BC, AGPCNP-BC, FNP-BC, or PA-C certification
- Fully onsite; able to travel between Sacramento area facilities
- New graduates welcome; 5+ years relevant RN experience also counts
- Background in geriatrics, skilled nursing, palliative care, or pain management preferred
- TEBRA or PointClickCare experience a plus
- Active Medicare credentialing strongly preferred

Schedule & Compensation

- $60–$100/hour + productivity bonus | ~$115,200–$192,000/year
- Full-time | ~32 hours/week | Schedule varies by facility
