We bring advanced wound care directly into the homes and care facilities of patients across California — many of whom are homebound, medically fragile, or simply exhausted by the toll that chronic wounds take on daily life.

Every visit is personal. Our team specializes in complex and chronic wound management, including skin substitutes and cellular/tissue-based products (CTPs), debridement, negative pressure wound therapy (NPWT), and UltraMIST ultrasonic-assisted therapy — but what really drives us is watching a wound that felt unmanageable finally start to close.

Role Description

\n


\n

• Administer UltraMIST ultrasonic-assisted wound therapy per physician/NP protocol

\n

• Perform wound care tasks within LVN scope: dressing changes, wound measurement/photo documentation, vital signs, supply management

\n

• Assist with telehealth visits- gather and document intake information, relay findings to the supervising NP/physician for evaluation

\n

• Maintain accurate, timely documentation in the EMR

\n

• Communicate wound status changes or concerns promptly to the NP/collaborating physician

\n

• Travel between patient homes/facilities in San Diego County

\n


\n

Qualifications

\n

\n

    \n
  • Active Licensed Practical Nurse (LPN) license in the state of California, with strong foundational Nursing skills.
  • \n

  • Demonstrated Wound Care experience, including assessment, treatment implementation, and ongoing evaluation.
  • \n

  • Background in Home Care or community-based Patient Care, with a focus on safety, comfort, and dignity.
  • \n

  • Ability to provide clear patient and caregiver education, and to collaborate effectively within a multidisciplinary team.
  • \n

  • Strong clinical documentation skills and basic proficiency with electronic medical records.
  • \n

  • Reliable transportation to patient locations in the San Diego area and ability to work a flexible part-time schedule.
  • \n

  • Current BLS/CPR certification; prior experience in wound care, rehabilitation, or long-term care settings is preferred.
