Our programs meet the needs of people, rather than demanding that people meet\n\nthe needs of our programs.\n\nJOB SUMMARY:\n\nCommunity Forward SF is hiring a Case Manager II. The Case Manager II primarily supports\n\nwomen living with HIV who participate in an approximately 18-month transitional housing\n\nprogram designed to support stability, health, and transition to permanent housing. The position\n\nalso provides case management and housing stabilization support to permanent supportive\n\nhousing (PSH) residents at Eddy Street Apartments.\n\nThe Case Manager partners with residents to identify strengths, needs, and self-defined goals and\n\nprovides person-centered support to promote health, housing stability, safety, and\n\nself-determination. Services include case management assessment and service planning,\n\ntherapeutic engagement, housing stabilization and transition support, advocacy, benefits\n\nassistance, care coordination, systems navigation, information and education, referrals, and\n\nconnection to community-based resources and providers.\n\nWe are looking for an experienced Case Manager with strong behavioral health knowledge,\n\nexcellent engagement and crisis de-escalation skills, and experience supporting adults with\n\ncomplex needs. The Case Manager builds collaborative relationships with residents, supports\n\nresident-identified goals, helps residents navigate housing, health care, behavioral health, benefits,\n\nand other systems, and recognizes when additional behavioral health, medical, crisis, or other\n\nspecialized support is needed.\n\nESSENTIAL JOB FUNCTIONS:\n\n● Establish and maintain a primary caseload of women living with HIV participating in the\n\ntransitional housing program and provide case management and housing stabilization\n\nsupport to PSH residents as assigned.\n\n● Build collaborative, trauma-informed relationships with residents using therapeutic\n\nengagement strategies that support trust, autonomy, self-determination, and progress\n\ntoward resident-identified goals.\n\n● Conduct intakes and case management assessments to identify resident strengths, needs,\n\npriorities, resources, and self-identified goals, and orient new residents to their housing\n\nprogram, program expectations, available services, staff, and community resources.\n\n● Meet regularly with residents to collaboratively develop, implement, and update Individual\n\nService Plans (ISPs) based on case management assessment and centered on\n\nresident-identified goals, strengths, needs, individual timelines, and personal choices.\n\n● Support transitional housing participants in identifying and working toward goals related\n\nto HIV and other health care, behavioral health, benefits, income, community connection,\n\nhousing readiness, and transition to permanent housing.\n\n● Support residents living with HIV in accessing and remaining engaged with HIV care and\n\ntreatment by identifying and addressing barriers such as transportation, scheduling,\n\nbenefits, stigma, and systems navigation and by coordinating with outside providers as\n\nappropriate.\n\n● Support PSH residents with housing retention and stabilization, including identifying and\n\naddressing barriers that may affect successful tenancy and connecting residents with\n\nappropriate resources and services.\n\n● Respond to crises with a calm, grounded presence; identify immediate safety concerns, use\n\nappropriate de-escalation strategies, follow established safety and crisis-response\n\nprotocols, and engage supervisors, behavioral health providers, mobile crisis services,\n\nemergency services, or other appropriate resources when additional intervention is\n\nneeded.\n\n● Recognize observable changes in behavior, functioning, or well-being that may indicate\n\nincreased behavioral health, substance use, medical, interpersonal violence, or other\n\nsafety concerns; respond within the Case Manager role, communicate concerns through\n\nestablished channels, and support connection to appropriate services.\n\n● Support residents in accessing and remaining connected to psychiatric, behavioral health,\n\nsubstance use, medical, and other health care services, including assistance with referrals,\n\nscheduling, transportation, communication with providers, and other barriers to care.\n\n● Coordinate with outside health, behavioral health, housing, benefits, and other service\n\nproviders when appropriate and authorized by the resident, including facilitating warm\n\nhandoffs and supporting continuity of care.\n\n● Maintain appropriate releases of information and protect resident privacy and\n\nconfidentiality, including heightened confidentiality requirements related to HIV and other\n\nhealth information.\n\n● Support residents in accessing and maintaining benefits, income, health insurance, housing\n\nresources, and other services that contribute to stability and resident-identified goals.\n\n● Support transitional housing participants with housing referrals and preparation for\n\npermanent housing, including obtaining and maintaining identification and other necessary\n\nhousing documentation and coordinating transitions and warm handoffs to permanent\n\nhousing and ongoing services.\n\n● Support PSH residents in maintaining stable tenancy, navigating housing-related systems\n\nand requirements, and addressing concerns that may affect housing stability, within the\n\nscope of the Case Manager role.\n\n● Plan, facilitate, or co-facilitate resident support groups, skill-building sessions, or\n\ncommunity activities consistent with the Case Manager role.\n\n● Assist residents in identifying and developing external support networks and community\n\nconnections.\n\n● Write clear, objective, timely, and strength-based case notes and complete required\n\ndocumentation in accordance with agency guidelines, contract requirements, and\n\napplicable database standards.\n\n● Maintain resident files and associated documentation with accuracy and appropriate\n\nprivacy protections.\n\n● Participate in weekly clinical case management supervision focused on strengthening\n\ntrauma-informed and therapeutic engagement, professional boundaries, and effective case\n\nmanagement responses to complex resident needs.\n\n● Coordinate regularly with teammates, supervisors, outside providers, housing partners,\n\nand community-based services as appropriate.\n\n● Maintain professional boundaries and follow established safety protocols while providing\n\nservices rooted in dignity, respect, choice, and trust.\n\n● Practice self-awareness, appropriate professional boundaries, and proactive\n\nstress-management strategies while working in a dynamic residential environment.\n\n● Participate in required trainings, team meetings, supervision, and other program activities.\n\n● Perform other duties as assigned consistent with the general scope of the position.\n\nBACKGROUND & EXPERIENCE\n\n● Bachelor’s degree or equivalent combination of education and relevant work experience.\n\n● At least two years of case management experience supporting adults with complex needs,\n\nsuch as serious mental health conditions, co-occurring substance use, chronic health\n\nconditions, HIV, and housing instability; experience in transitional housing, permanent\n\nsupportive housing, residential services, behavioral health, HIV services, or similar settings\n\npreferred.\n\n● Strong working knowledge of behavioral health, including common mental health\n\nsymptoms, co-occurring substance use, overdose risk and prevention, and the ways\n\nbehavioral health needs may affect functioning, engagement, and housing stability.\n\n● Demonstrated skill in trauma-informed and therapeutic engagement while maintaining\n\nappropriate professional boundaries and the scope of the Case Manager role.\n\n● Demonstrated crisis-response and de-escalation skills, including the ability to identify\n\nimmediate safety concerns, follow established protocols, and exercise sound judgment\n\nabout when to involve supervisors, behavioral health providers, mobile crisis teams,\n\nemergency services, or other appropriate resources.\n\n● Experience conducting case management assessments and collaboratively developing\n\nservice plans based on individual strengths, needs, priorities, and self-identified goals.\n\n● Knowledge of HIV care and the practical, social, and systemic barriers that may affect\n\nconsistent engagement in care and treatment.\n\n● Understanding of homelessness, trauma, DV/IPV, harm reduction, neurodiversity,\n\ncognitive impairment, LGBTQIA+ communities, and systems of oppression.\n\n● Understanding of trauma-informed practice, person-centered support, and cultural\n\nhumility, including stigma-free, affirming support for women living with HIV and for\n\ncisgender and transgender women.\n\n● Understanding of the role and boundaries of case management, including when resident\n\nneeds require connection to clinical, medical, crisis, or other specialized services.\n\n● Knowledge of confidentiality requirements for health information, including HIV-related\n\ninformation.\n\n● Experience coordinating effectively with healthcare, behavioral health, housing, benefits,\n\nand other community-based providers.\n\n● Demonstrated ability to maintain a calm, grounded presence during high-stress situations\n\nand practice compassionate conflict de-escalation.\n\n● Effective written and verbal communication skills.\n\n● Strong organizational, documentation, and time-management skills.\n\n● Comfort with technology, including MS Office, Google Suite, and applicable city databases\n\nsuch as ONE.\n\n● Knowledge of San Francisco and Bay Area resources.\n\n● Ability to work both independently and collaboratively as part of a team.\n\n● CPR and First Aid certification within 90 days of hire.\n\n● TB clearance within 7 days of hire.\n\nPREFERRED QUALIFICATIONS\n\n● Multicultural and/or multilingual.\n\n● 40-hour Domestic Violence Advocate certification.\n\n● Training in crisis intervention, de-escalation, suicide prevention or suicide-risk\n\nidentification, such as Mental Health First Aid or Nonviolent Crisis Intervention.\n\n● Experience supporting women living with HIV.\n\n● Experience working in transitional housing and/or permanent supportive housing.\n\n● Lived experience with homelessness, recovery, or systems of oppression.\n\n● Strong self-awareness, cultural humility, and commitment to reflective practice.\n\n● Excellent organizational and administrative skills.\n\n● Knowledge of San Francisco community resources and neighborhoods, including the\n\nCoordinated Entry system.\n\nPHYSICAL REQUIREMENTS:\n\n● Active Milieu: Supportive housing environments are dynamic. Candidates should feel\n\ncomfortable navigating an active environment where unexpected resident interactions,\n\nvaried noise levels, and changing day-to-day priorities occur.\n\n● Physical Mobility: Ability to move through multi-story residential buildings (commonly\n\nincluding stairs), walk between program locations, and accompany residents out in the\n\ncommunity.\n\n● Standard Tasks: Ability to sit and use standard computer equipment (keyboard, visual\n\ndisplay terminal) for extended periods to complete documentation.
