Implement PT treatment plans for SNF residents

Provide skilled therapy services under PT supervision
</li>

Document patient progress and interventions
</li>
<li>
Collaborate with interdisciplinary care team
</li>
<li>
Maintain productivity and quality standards
</li>
</ul>
Requirements
<ul>
<li>
Active Physical Therapy Assistant (PTA) license
</li>
<li>
SNF or post-acute rehab experience required
</li>
<li>
Strong communication and teamwork skills
</li>
<li>
Knowledge of documentation and compliance standards
</li>
