American Traveler is seeking a Home Health RN Care Manager requiring a California RN license and EPIC experience for a day shift skilled nursing role.

- Manages a patient caseload, coordinating care across disciplines
- Day shift schedule, 8:00 AM to 4:30 PM
- EPIC experience required

Requirements\n

- Active California RN license required
- BLS/CPR certification from an approved vendor required
- Valid driver's license required, candidate must be at least 21 years old
- Current automobile insurance and a reliable personal vehicle required
- BSN preferred
- Acute care facility or home care experience preferred
- Candidates with a prior adverse licensure action will not be considered

- Performs initial and ongoing comprehensive patient assessments and updates plans of care as needed
- Educates patients and family members on healthcare and nursing needs
- Documents patient care, status, and response to treatment in accordance with policy

- Complies with HIPAA and all applicable documentation standards
- No more than 5 days of requested time off (RTO) will be considered
- Local candidates accepted with no radius restriction; local pay rates match travel rates
