We are looking for a strong, compassionate Case Manager who understands and respects the military culture. The ideal candidate will have military experience — either as a Veteran themselves or someone who has grown up around and understands the military lifestyle and culture.

Responsibilities:

Provide case management and residential support to Veterans

Assist residents with service plans, housing goals, and independent living skills

Connect Veterans with VA, medical, social, educational, and community resources

Assist with housing searches, appointments, outreach, and discharge planning
-
Facilitate structured groups and living skills activities
-
Monitor residents’ progress and well-being
-
Maintain accurate documentation and progress notes

Collaborate with VA personnel, families, providers, and the care team

Assist residents with daily living activities when needed
Qualifications:
-
High School Diploma/GED required
-
Bachelor’s degree in Social Work or a related field preferred

Two years of related experience preferred
-
Military or Veteran experience strongly preferred
-
Knowledge of Veteran services and community resources is a plus
-
Valid First-Aid certification required
-
Must pass a criminal background check
-
Must be at least 18 years old
