Job Title: Primary Care Physician
Location: North Hollywood, California
Job Type: Full-Time, W-2
Work Setting: PACE Center with community-based care and home visits
Schedule: Monday through Friday, 8:00 AM to 5:00 PM, with rotating phone-only call a few nights or weekends per month
Pay: $275,246 - $330,595 base salary, plus quarterly bonus eligibility up to 10% of salary

Responsibilities
- Perform comprehensive in-person assessments for program participants
- Lead and partner with the interdisciplinary team to develop individualized care plans
- Provide ongoing primary care services in a PACE setting
- Coordinate discharges for acute, short-term, and long-term placements
- Support continuity of care across clinic, community, and home-based settings
- Participate in a rotating phone-only call schedule
- Deliver high-quality care in a value-based environment with an average of 8 patients per day
Required Qualifications
- Doctorate degree
- Successful completion of an ACGME-accredited residency program
- Active physician license in the state of practice without restrictions
- State CDS certification and DEA registration
- Eligible for enrollment to provide services to Medicare and Medi-Cal participants
- Background in Internal Medicine, Family Medicine, Emergency Medicine, or Geriatrics
- 2 to 5 years of relevant clinical experience
Preferred Qualifications
- Board certification
- Experience in geriatric-focused primary care
- Experience working within an interdisciplinary care team
- CPR and First Aid certification upon hire preferred, or ability to obtain within 180 days of hire
