Implement PT treatment plans for SNF residents
</li>
<li>
Provide skilled therapy services under supervision
</li>
<li>
Document patient progress and interventions
</li>
<li>
Support interdisciplinary care delivery
</li>
<li>
Maintain productivity and quality standards
</li>
</ul>
Requirements
<ul>
<li>
Active Physical Therapy Assistant (PTA) license
</li>
<li>
SNF or post-acute rehab experience required
</li>
<li>
Strong communication and teamwork skills
</li>
<li>
Knowledge of therapy documentation standards
</li>
</ul>
