About the Role:
Join MedPOINT Management as a Recovery Specialist, where you'll play a crucial role in helping patients navigate their recovery journey. We are dedicated to providing exceptional support and care, ensuring our clients receive the best possible outcomes.
Responsibilities:
- Conduct comprehensive assessments to evaluate patient recovery needs.
- Develop individualized recovery plans in collaboration with healthcare teams.
- Monitor patient progress and adjust plans as necessary to ensure optimal recovery.
- Provide emotional support and guidance to patients and their families.
- Educate patients on recovery techniques and resources available to them.
- Document patient interactions and maintain accurate records in compliance with regulations.
- Coordinate with medical professionals to facilitate seamless care transitions.
- Participate in team meetings to discuss patient cases and share best practices.
Requirements:
- Bachelor's degree in a related field; relevant certifications are a plus.
- 1-3 years of experience in recovery, rehabilitation, or a related healthcare setting.
- Strong interpersonal skills and the ability to connect with diverse patient populations.
- Excellent organizational and time management skills.
- Knowledge of recovery processes and patient care best practices.
- Ability to work collaboratively within a multidisciplinary team.
- Compassionate demeanor and a passion for helping others.
- Proficient in using electronic health record (EHR) systems.
