We are currently hiring a Registered Nurse for a Part-Time Hybrid RN Care Coordinator position. Join our team and make a meaningful difference in the lives of older adults and their families. In this role, you will coordinate client care, guide and mentor our CNAs, and support their professional growth while helping ensure compassionate, quality care for our clients.
This position combines the flexibility of working remotely from home with required in-person responsibilities, including client assessments and attendance at our in-person Orientation Classes every other week. Each client will receive a minimum of four in-person assessments per year.
As an RN Care Coordinator, you will play an important role in ensuring quality care, maintaining effective communication, and supporting our team. Your responsibilities will include:
- Act as a primary point of contact for the client / family for nursing related questions/ concerns.
- "On-duty" hours for this position will be during hours of 7a-7p, during which time the client / family can contact you as needed. We do ask that you are readily available to take calls/emails during "on-duty" hours, meaning that you are able to respond in no more than 30-minutes to each inquiry.
- Outside of your "on-duty" hours, communication will be limited to emergency situations only.
- Conduct the initial consultation and 90-day reassessments, including nursing assessment and care plan.
- Outside of the in-person assessments, the remainder of the job can be completed from home. Initial assessments are typically done within 24-48 hours of request, so you will need to have some schedule flexibility to accommodate short notice requests.
- Communicate with and supervise all Caregivers (CNAs) on their daily duties (assist clients with daily living activities, provide mobility support, prepare meals, assist with feeding, housekeeping, assist with errands) providing direction, instruction, and mentorship as needed
- Attend virtual team meetings every two weeks (required)
- Attend in-person Orientation Classes every two weeks (required)
- You will be tasked to manage the care process, instruct caregivers, find solutions to issues and concerns that family members have related to your client(s) plan of care, and generally ensure that the care process runs smoothly. The main goal of this position is to ensure the success of the relationship between the caregiver(s) and the client. Solve problems where they exist, provide coaching and training, and support both parties emotionally and strategically.
Job Requirements
Ideal candidates will have:
- Active California RN License in good standing.
- Reliable transportation and valid Driver's License.
- Strong communication and interpersonal skills.
- Strong organizational and time-management skills, with the ability to manage multiple priorities.
- Ability to work independently and exercise sound clinical judgment.
- Strong attention to detail and accurate documentation skills.
- Strong computer and technology skills, including proficiency with Gmail, Google Drive, Google Workspace, mobile applications, and electronic documentation systems.
- Ability to learn and adapt to new software and technology quickly.
- Compassionate and supportive approach to mentoring caregivers, with a willingness to provide guidance, coaching, and constructive feedback to help them succeed in their role.
- Professional and compassionate approach when communicating with clients, families, caregivers, and healthcare professionals.
- Ability to be flexible and responsive to changing client and staffing needs.
Preferred:
- Previous experience in home care, home health, or care coordination
Compensation
- You will be paid $125 per case on a bi-weekly basis. This covers all aspects of managing the case. You will start with up to 3-5 cases and increase as desired.
