Physical Therapist (PT)
Split Position – Valley Oaks Post Acute & Santa Maria Post Acute
This role is shared between two facilities located just 2 miles apart. The selected candidate will work a total of 35–40 hours per week across both locations.

Valley Oaks Post Acute is seeking a compassionate Physical Therapist (PT) to join our in-house rehabilitation team.
This is an excellent opportunity for a PT who is passionate about helping residents improve mobility, strength, balance, and overall independence. As part of our collaborative rehabilitation team, you will provide personalized, resident-centered therapy and develop individualized treatment plans that support meaningful progress, improved function, and a higher quality of life. Key Responsibilities

Evaluate residents’ mobility, strength, balance, and functional needs
-
Develop and implement individualized physical therapy treatment plans
-
Provide skilled therapy to reduce pain, restore function, and promote healing
-
Monitor resident progress and adjust treatment plans as needed

Educate residents, families, and caregivers
-
Collaborate with the interdisciplinary care team

Maintain accurate, timely, and compliant documentation
-
Ensure services meet all professional, legal, and regulatory standards
Qualifications
-
Active Physical Therapy state license in good standing

Skilled nursing or long-term care experience preferred

Strong clinical, communication, and organizational skills

Knowledge of the Net Health Electronic Documentation System preferred
