Duration: 26 Weeks
Weekly Pay: $2,752

Requirements:

Minimum 2 years of acute-care hospital case management experience

Experience in care coordination and discharge planning

Epic charting experience
Assignment Details:

Every other weekend required

No call required

Floating between like units based on facility needs
