Job DescriptionSeeking a Full Time Licensed Vocational Nurse (LVN) Case Manager for our growing hospice agency
You will be responsible for the management and treatment of assigned patients. Hospice experience is desired. The ideal candidate will be a responsible and well-trained professional able to give the best nursing care with little supervision. We need a compassionate and cool-tempered team member. You will be able to follow health and safety guidelines faithfully and consistently. You will coordinate the hospice care plan, supervise staff, and provide skilled nursing hospice and palliative care services to our patients. The nurse will work cooperatively with the attending physician, Hospice Medical Director, and other members of the interdisciplinary Hospice team. The patient care coordinator will adhere to the plan of care, perform and manage prescribed medical treatments (including pain management and symptom control), conducts assessments and evaluations and provides education and supportive care to patient and caregivers. Must be able to utilize computers and be comfortable with EMR (Hospice MD)
QUALIFICATIONS:
Current Nursing license within the State.
Hospice experience required.
Responsibilities
- Monitor patients condition and assess their needs to provide the best possible care and advice
- Observe and interpret patients symptoms and communicate them to physicians
- Collaborate with physicians and nurses to devise individualized care plans for patients
- Perform routine procedures (blood pressure measurements, administering injections etc.) and fill in patients charts
- Adjust and administer patients medication and provide treatments according to physicians orders
- Inspect the facilities and act to maintain excellent hygiene and safety (decontaminating equipment, sanitizing surfaces, preparing beds etc.)
- Provide instant medical care in emergencies
- Assist surgeons during operations
- Foster a supportive and compassionate environment to care for patients and their families
- Expand knowledge and capabilities by attending educational workshops, conferences etc.
Skills
- Proven experience as a supervisor
- Excellent knowledge of nursing care methods and procedures
- Excellent knowledge of emergency care
- In-depth knowledge of health and safety guidelines and procedures (sanitation, decontamination etc.) and willingness to follow them at all times
- A team player with excellent communication and interpersonal skills
- Responsible and compassionate
- Outstanding organizational and multi-tasking skills
- Patience and excellent problem-solving skills
- Valid nursing license
Job Type: Full-time
Pay: $32.00 - $40.00 per hour
