Leisure Glen Post Acute Care Center in Glendale, California, is looking for a motivated and compassionate Physical Therapist (PT) to join our rehabilitation team!
We have a wonderful team of therapists, a welcoming work environment, a convenient Glendale location, and plenty of on-site parking.
We are especially interested in Physical Therapists who are available to work weekends! Full-time and per diem opportunities are available.
Whether you're an experienced PT or a new graduate looking to grow your clinical skills, we'd love to hear from you!
Responsibilities

Evaluate and treat residents with a variety of orthopedic, neurological, and functional mobility needs.

Develop individualized treatment plans to improve strength, balance, mobility, and independence.

Provide skilled physical therapy interventions to support rehabilitation and recovery.

Complete accurate and timely evaluations, progress reports, and treatment documentation.

Collaborate with our experienced rehabilitation team, nursing staff, and other interdisciplinary team members.

Educate residents and families on mobility, safety, fall prevention, and therapeutic exercises.

Participate in discharge planning and interdisciplinary care meetings.
Qualifications

Current California Physical Therapist license.

Strong clinical reasoning, communication, and teamwork skills.

Skilled nursing facility experience preferred, but not required.
