- Schedule Type: Clinic and Call
- Call Type: Weeknight Beeper and 24 Hour Weekend Beeper Call
- Dates of Need: we need ongoing coverage about 1-2 weeks a month
- Shift Times: See below in additional details
- Lunch N/A
- Provider Min # Shifts Required: 5-7 shifts a month
- Guaranteed Hours per Day: 8
- Hours Worked for OT to apply: 12 hours per day
- Rates to be Filled Out on Bid: Clinic and Call: Regular, Orientation, Overtime, Holiday, Night Call, 24 Hour Call, Call-Back.

\n Please note the following travel deviations from our contract will apply to the assignment (or current confirmation agreement):

- NOT reimbursed:

o Mileage from provider’s home to departing airport, as well as from airport to home upon return.
\n o Ride share (Lyft/Uber/etc.) and/or taxi from provider’s home to departing airport, as well as from airport to home upon return.

- WILL be reimbursed:
\n
\n
o Airport parking at provider’s departing airport if provider drives themselves to the airport.
\n o Insurance policies for rental cars must be pre-approved, prior to booking, otherwise it will not be reimbursed.
\n o Ride share (Lyft/Uber/etc.) and/or taxi from destination airport to hotel/lodging for assignment, as well as from lodging to hospital/clinic, and return trips.
