Responsibilities

• Provide comprehensive family medicine clinical care to patients on-site.

• Deliver evidence-based treatment plans with measurable outcomes.

• Engage in continuous learning to advance clinical skills and certifications.

• Utilize patient pipelines and data to forecast care needs and targets.

• Meet or exceed individual performance targets and contribute to territory goals.

• Maintain accurate, timely documentation in electronic health records.

• Collaborate with a multidisciplinary team to coordinate patient care.

• Engage in-site patient outreach and follow-up to ensure continuity of care.

• Adhere to regulatory and safety standards to ensure high-quality care.

• Participate in regular case reviews and quality improvement initiatives.

• Educate patients on preventive care and lifestyle modifications.

• Represent the organization professionally in the community and within the clinic.

Experience/Qualifications

• Board-certified/eligible Family Medicine physician with state license

• Deliver evidence-based care with measurable patient outcomes

• On-site patient care with ability to manage full-scope family medicine

• Compliance with regulatory and safety standards (HIPAA, quality metrics)

• Strong EMR documentation skills with timely chart entries

• Data-driven care planning using patient pipelines and forecasts

• Participation in continuous learning and ongoing professional development

• Collaborative team player in multidisciplinary care teams

• Patient education on preventive care and lifestyle modification

• Strong patient outreach and follow-up to ensure continuity of care
