We are a privately owned home health and hospice provider that has served Los Angeles County for more than 15 years. Home health operations began in 2008, with hospice following in 2010. We are an ethics-driven organization with deep institutional relationships across the region, fully credentialed and contracted with leading hospital systems, and known for clinical excellence and transparent, compliant operations.

- 70 to 80 field nurses and clinicians
\n
- Payors include Medicare, Medi-Cal, and LA Care
\n
- Full Los Angeles County coverage
\n
- Care delivered in the home, skilled nursing facilities, assisted living, and board and care settings
\n
- No inpatient hospice facility; we go where the patient is

THE OPPORTUNITY
\n
\n
We are hiring a Director of Business Development / Senior Liaison to drive continued growth across hospital systems and skilled nursing facilities throughout LA County.
\n
\n
This is a senior, relationship-driven role for someone with established referral relationships who wants to align with a credible, long-standing organization. We are hiring at a level that fits the person. If you are a senior liaison with a strong referral base and want the runway to build a team, that works. If you are already a director ready to own growth strategy, that works too.

WHAT YOU WILL DO
\n
\n
- Generate consistent referrals from hospitals, skilled nursing facilities, and case management teams
\n
- Build and deepen relationships with hospital case managers, discharge planners, SNF leadership, care coordination teams, and physicians
\n
- Represent hospice services across LA County
\n
- Serve as a senior face of the organization in institutional settings
\n
- Identify growth opportunities within existing hospital systems and SNF networks
\n
- Maintain alignment with CMS, Medicare, and state compliance requirements
\n
- Partner with clinical leadership to ensure seamless patient transitions
\n
- Contribute to strategic planning and build out a business development team as we scale
\n
\n

WHAT YOU BRING

- 3+ years of business development or liaison experience in hospice, home health, or both
\n
- Direct experience and active relationships in Los Angeles County
\n
- Established referral relationships across hospitals and skilled nursing facilities that you can speak to in numbers
\n
- Strong working knowledge of Medicare hospice and home health regulations
\n
- Familiarity with hospital discharge workflows and case management referral processes
\n
- A track record of building trust with institutional partners
\n
- Entrepreneurial mindset with leadership ambition
\n
- Valid driver's license and reliable transportation for daily field work
\n
- Relationships with LA physician groups or IPAs
\n
\n
