Position Overview
\n
We are seeking a Physical Therapist (PT) for a 13-week assignment primarily supporting a Skilled Nursing Facility (SNF)/Extended Care Center (ECC). The PT will provide rehabilitation services to both short-term rehab patients and long-term care residents. Based on departmental needs, the therapist may also assist within the hospital's inpatient acute care setting.

Responsibilities

- Evaluate and treat SNF rehabilitation patients and residents.
- Develop and implement individualized therapy plans.
- Collaborate with Occupational Therapy and the interdisciplinary care team.
- Participate in weekly patient care conferences.
- Contribute to discharge planning and care recommendations.
- Maintain accurate documentation in EPIC and Therapute.
- Flex to the inpatient department as needed.

Requirements

- Active Physical Therapist license
- Minimum 1-2 years of PT experience
- Skilled Nursing Facility and rehabilitation experience preferred
- Strong interdisciplinary communication skills
