Social Worker Opportunity:
\n

- Full-time, permanent position
- Monday-Friday, working 8:00AM-5:00PM
- The Social Worker will assess patients' psychosocial needs and provide counseling, emotional support, and individualized care planning
- Connect patients and families with community resources while advocating for their social, financial, and support needs
- The Social Worker will collaborate with the interdisciplinary care team to coordinate care, maintain documentation, and support quality patient outcomes

Social Worker Requirements

- Master’s Degree in Social Work Required
- LCSW's are preferred but LMSW's, CSWA's, and ACSW's are welcome to apply
