Local: $54–$58/hr

Position Overview

We are seeking an experienced Registered Nurse Case Manager with acute-care case management experience to assess patient needs, coordinate care plans, and manage discharge planning. The ideal candidate will have previous California case management experience and an active California RN license.

Responsibilities

• Assess patients' clinical and psychosocial needs and coordinate appropriate care plans.

• Coordinate patient care with physicians, nursing staff, and interdisciplinary teams.

• Manage discharge planning and facilitate transitions to appropriate post-acute care settings.

• Coordinate referrals and other services required for continuity of care.

• Monitor patient progress and support appropriate utilization of healthcare resources.

• Maintain accurate case management records and documentation.

• Communicate with patients, families, providers, and external healthcare organizations.

Required Qualifications

• Minimum 2 years of acute-care case management experience.

• Previous California case management experience.

• Active California Registered Nurse license at the time of submission. Pending licenses are not accepted.

• Current Basic Life Support certification.

• Mandatory flu vaccination; no exemptions accepted.

Preferred Qualifications

• EPIC electronic health record experience.

• Strong care coordination and discharge planning skills.

• Experience working in an acute-care hospital environment.

Scheduling Requirements

• No weekend, floating, or on-call requirements.

• Scheduling requests cannot be accommodated.
