\n 🩺 Position Overview
\n We are seeking an experienced Physical Therapist (PT) to provide compassionate, patient-centered rehabilitation services in a Skilled Nursing Facility setting. The ideal candidate will help patients improve mobility, strength, functional independence, and overall quality of life through individualized treatment plans.
\n ✅ Requirements
\n 🎓 Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program
\n 📋 Active California PT License required
\n 💪 Minimum 3 years of Physical Therapy experience preferred
\n 🏥 Skilled Nursing Facility or similar rehabilitation experience preferred
\n 🧠 Strong knowledge of therapeutic exercises, modalities, and adaptive equipment
\n 🤝 Excellent communication and interdisciplinary collaboration skills
\n ❤️ Compassionate, patient-focused approach to care
\n 📌 Responsibilities
\n ✔️ Perform comprehensive physical therapy evaluations
\n ✔️ Develop and implement individualized treatment plans
\n ✔️ Provide therapeutic interventions to improve strength, mobility, and function
\n ✔️ Monitor patient progress and modify treatment plans as needed
\n ✔️ Educate patients and families on exercises, safe mobility, and home modifications
\n ✔️ Collaborate with nurses, physicians, occupational therapists, and other members of the care team
\n ✔️ Maintain accurate and timely clinical documentation
\n ✔️ Ensure treatment plans comply with facility policies and regulatory standards
