PRN Physical Therapist | Altadena, CA

$55–$65/hour | W-2 | Flexible Schedule

Evergreen Rehabilitation is looking for a PRN Physical Therapist to join our growing team serving older adults in independent living, assisted living, and memory care communities.
\n
We’re looking for a PT who is passionate about geriatric care, functional rehabilitation, and helping older adults age in place.
\n
\n
What you’ll do:
\n
• Perform PT evaluations, progress notes, re-evaluations, and skilled treatments
\n
• Address balance, gait, strength, mobility, orthopedic, neurological, and age-related conditions
\n
• Incorporate therapy into the resident’s real-life environment — apartments, hallways, gyms, pools, and community spaces
\n
• Focus on practical, functional goals that support independence and quality of life
\n
• Collaborate closely with OT, nursing, caregivers, physicians, and other members of the care team
\n
• Complete timely, accurate documentation
\n
\n
What we’re looking for:
\n
• Active California PT license
\n
• CPR/BLS certification
\n
• Own professional liability/malpractice insurance
\n
• Strong geriatric and clinical skills
\n
• Excellent interdisciplinary communication
\n
• Reliable, independent, and comfortable managing your own caseload
\n
\n
Schedule: Morning, daytime, evening, and weekend availability are all welcome (per the clients necessity). We’re looking for someone who can provide consistent availability while maintaining flexibility.
