- Salary: $158,000 – $185,000 annually (Based on experience) + Benefits
Position Overview
We are seeking a dedicated and compassionate Family Nurse Practitioner (FNP) to join a multidisciplinary healthcare team providing comprehensive medical services to patients at a community health center in San Jose.
The ideal candidate will have strong primary care experience and a passion for providing high-quality, patient-centered care, particularly within diverse and underserved communities.
-> Key Responsibilities
- Provide comprehensive primary care services, including assessment, screening, diagnosis, treatment, and preventive healthcare.
- Conduct physical examinations and obtain comprehensive patient medical histories.
- Develop and implement individualized treatment plans based on patient needs.
- Prescribe and manage medications and monitor patient outcomes.
- Educate patients on disease prevention, medication adherence, healthy lifestyles, and overall wellness.
- Collaborate with physicians, behavioral health providers, and other interdisciplinary team members to coordinate patient care.
- Maintain accurate, timely, and complete documentation in the electronic health record (EHR).
- Coordinate referrals and consultations with specialists.
- Participate in team meetings and support team-based clinical operations.
- Provide clinician cross-coverage as needed.
- Participate in an after-hours on-call rotation.
-> Qualifications
- Master's degree or Doctorate from an accredited Nurse Practitioner program.
- Current, unrestricted California Nurse Practitioner license.
- Current board certification as a Nurse Practitioner.
- Active and unrestricted DEA license.
- Current BLS certification.
- Minimum 2 years of primary care clinical experience required.
- Experience in a community health center, FQHC, or underserved healthcare setting preferred.
- Strong clinical, communication, and patient-care skills.
- Ability to work collaboratively within a multidisciplinary healthcare team.
