As an LPN, you will play a crucial role in our high-touch, team-based medical care, ensuring our participants receive personalized and integrated support.
Key Responsibilities
- Provide direct nursing care under the supervision of a registered nurse or physician.
- Administer medications and treatments as prescribed.
- Monitor and document participant vital signs and health status.
- Assist with participant assessments and care plan development.
- Communicate effectively with participants, families, and the interdisciplinary team at myPlace Health.
- Educate participants and caregivers on health maintenance and disease prevention.
- Maintain accurate and detailed medical records.
- Respond to emergent situations in a calm and efficient manner.
Required Qualifications & Experience
Required:
- Minimum 2 years of experience as a Licensed Practical/Vocational Nurse.
- Ability to work overtime and night shifts.
- Current LPN/LVN license in the state of California.
