Public Health Nurse (PHN)
Location: Bakersfield, CA
Compensation: $38–$58 per hour, DOE
Employment Type: Direct Hire | Full-time | Non-exempt
Schedule: Monday–Friday, 40 hours per week
Work Arrangement: 100% onsite
Travel: Approximately 25% for outreach, events, trainings, and other organizational needs
About the Opportunity
Key Staffing is seeking a Public Health Nurse for a community-focused healthcare organization in Bakersfield, CA. This direct-hire opportunity is ideal for a Registered Nurse interested in preventive care, health education, case management, and public health program administration.
The position emphasizes care coordination, assessments, quality assurance, reporting, and program development. Approximately 25% of the role may involve direct patient or client interaction, including screenings, education, and occasional home visits.
The Public Health Nurse will collaborate with nursing staff, medical providers, behavioral health professionals, and outreach and case management teams. The ideal candidate brings strong organizational skills, cultural sensitivity, and a long-term interest in serving American Indian and Alaska Native communities.
Key Responsibilities
- Complete health screenings and intake assessments for adults and children, addressing general health, women’s health, chronic conditions, and HIV/AIDS risk assessment and prevention.
- Develop and coordinate individualized care plans with a multidisciplinary team.
- Educate clients about their healthcare needs, preventive services, chronic disease management, and available community resources.
- Coordinate referrals to primary care, specialty care, and other services to support continuity of care.
- Conduct face-to-face evaluations, educational encounters, screenings, and home visits as required by program or funding requirements.
- Coordinate and oversee case management and outreach activities to support appropriate treatment plans and follow-up.
- Support diabetes care management and oversee diabetes program activities, grant requirements, and deliverables.
- Monitor public health program goals, performance measures, and outcomes; recommend improvements to services and processes.
- Conduct chart reviews and support quality assurance initiatives.
- Maintain accurate electronic health records, including encounter documentation and applicable ICD-10 and CPT coding.
- Maintain client tracking information to support outreach and home visit scheduling.
- Prepare required monthly reports, program records, and grant-related documentation.
- Assist with grant writing, proposals, and the development or revision of clinical protocols, policies, and procedures.
- Participate in community events, outreach activities, trainings, and organizational meetings.
- Keep leadership informed of program activities, pending concerns, and potential issues.
- Provide culturally responsive care and education while demonstrating respect for the communities served.
Required Qualifications
- Bachelor of Science in Nursing (BSN).
- Active California Registered Nurse license.
- Valid California Public Health Nurse certification.
- Minimum of two years of experience as a Registered Nurse.
- Valid California driver’s license.
- Experience using electronic health records and clinical information systems.
- Proficiency with Microsoft Office applications, including Outlook, Teams, and Excel.
- Strong written and verbal communication skills, including the ability to explain health information to individuals and groups with varied educational and cultural backgrounds.
- Knowledge of communicable disease prevention, transmission, and control, along with common medical conditions and care coordination practices.
- Strong organizational skills and the ability to prioritize responsibilities, work independently, and collaborate across departments.
- Cultural sensitivity and a willingness to learn about and respectfully support American Indian and Alaska Native communities.
- Ability to work onsite and participate in required travel.
Preferred Qualifications
- At least two years of public or community health experience.
- At least one year of diabetes care management experience.
- Experience in case management, population health, community outreach, or preventive health education.
- Experience with grant-funded programs, grant reporting, grant writing, or program management.
- Familiarity with public health performance measures and reporting requirements.
- Experience developing policies, procedures, or clinical protocols.
- Experience with Greenway electronic health records.
