Position Summary

The Community Mental Health Counselor II (CMHC II) provides comprehensive case management, rehabilitation, crisis intervention, counseling support, and care coordination services to adults living with serious mental illness and/or co-occurring disorders. Working within a multidisciplinary treatment team, the CMHC II promotes recovery, independence, community integration, and improved quality of life through person-centered, culturally responsive, and trauma-informed care.

Essential Duties and Responsibilities

  • Provide intensive case management services for an assigned caseload.
  • Develop, implement, and monitor individualized treatment plans.
  • Coordinate behavioral health, medical, housing, employment, educational, and community resources.
  • Provide rehabilitation, recovery support, crisis intervention, and de-escalation services.
  • Facilitate psychoeducational and rehabilitation groups.
  • Maintain timely, accurate electronic health record documentation.
  • Participate in interdisciplinary treatment team meetings.
  • Assist clients with appointments, benefits, housing, transportation, and community integration.
  • Ensure compliance with DHCS, LACDMH, HIPAA, CARF, and agency standards.
  • Perform other duties as assigned.

Minimum Qualifications

  • Bachelor's degree in Psychology, Social Work, Sociology, Human Services, Behavioral Science, Counseling, or a related field. Associate degree plus 2 years of experience.
  • One (1) to three (3) years of behavioral health or case management experience preferred.
  • Valid California Driver License and acceptable driving record.
  • Successful DOJ/FBI background clearance and health screening.

Knowledge, Skills & Abilities

  • Knowledge of serious mental illness and recovery-oriented care.
  • Strong communication, documentation, and organizational skills.
  • Ability to work effectively with diverse populations.
  • Proficiency with Electronic Health Records and Microsoft Office.

Physical Requirements

Ability to sit, stand, walk, bend, lift up to 25 pounds, and travel within the community as required.
