Requirements\n

- Active California RN License
- 2+ years of current Acute Care Hospital Case Management experience (Care Coordination & Discharge Planning required)
- Epic experience
- Previous travel experience preferred (strong first-time travelers may be considered)
- Experience with HMOs/IPAs, Advance Directives, POLST, and Protective Services
