Job Description:

This position involves facilitating patient transfers from hospitals within and outside of Dignity Health. The role includes managing cases after the intake process, performing patient assessments, and coordinating with MDs and specialists to request bed placements at appropriate facilities.

Responsibilities:

- Facilitate transfers from various hospitals, ensuring the acceptance of physicians to take the patient.
- Manage patient assessments, vitals, and clinical information post-intake process.
- Coordinate with a list of hospitals and request bed placements.

Requirements:

- Minimum of 2 years of ICU, ED, or transfer center experience.
- Acute care experience.
- Critical care assessment skills and the ability to multitask.
- Proficiency in Cerner, TeleTracking, and various Google applications.
- BLS Certification required.
